* Cancellation Window- Cancellation must be made at least 1 hour in advance before booked time. Late cancellations or no-shows will be charged the full booking amount. * Late cancellations - late cancellations will receive no refund. And will be charged the full rate. * No shows- No shows will be charged the full rate. No refund will be available. * WE DO NOT REFUND FOR FORGETTING TO CANCEL A MEMBERSHIP * How Cancellations are made- When canceling a booking, Customer must email Pinky’s Tee Box letting them know of the cancellation. The email will help timestamp the cancellation to be sure you canceled within the allotted time. ACKNOWLEDGMENT I have read and fully understand this Agreement, including the waiver and release of significant legal rights. I accept these terms freely and voluntarily.

* Booking Structure- Minimum booking time is 30 minutes. Max time booking for a reservation is a 2 hour limit. * Add-on- When your original booking time is coming to an end, if there is no one booked behind you, the option to book additional time is available. An automated text will be sent to remind you of your time being almost over. At that time if no one is booked behind you, additional time can be added * Guests:- Guest limit is 4 per booking * Start/stop time & late arrival: Booking time starts and stops on time. Time is not extended for late arrivals. There are no refunds for ending early. * Equipment rules: Use Clean clubs only. No Metal spikes. Use provided balls ONLY!! * If Any balls, tees or any other equipment provided by Pinky’s Tee Box is takin by a customer, they are subject to be charged extra money for equipment replacement. * Damage- Customer/Member are reliable and will pay for any and all damages done during their reservation time. The customer on file for the booking will be charged for equipment damage accordingly. The customer will be accountable for all damages and misuse from the guests. * Alcohol: NO ALCOHOL IS ALLOWED! * Customers may bring food or drinks in with them, as long as the drinks do not contain any alcohol. * No food or drinks in the simulator area or near computer. MINOR LIABILITY WAIVER AND PARENT/GUARDIAN CONSENT
 Pinky’s Tee Box Golf Simulator * Parent/Guardian ( for minors)- If signing/agreeing on behalf of a minor, I certify that | am the parent, legal guardian, or authorized custodian of the minor. I agree to indemnity and hold harmless Pinky’s Tee Box from any claims arising from the minor's participation. * 1. Assumption of Risk I, the undersigned parent or legal guardian, understand and acknowledge that use of the golf simulator and related facilities involves inherent risks, including but not limited to bodily injury, property damage, or death. These risks may result from swinging golf clubs, being struck by golf balls, equipment malfunction, or the actions of the minor or others. 2. Parental Consent I give permission for the above-named minor to participate in activities at Pinky’s Tee Box and use its golf simulator and related facilities. 3. Release of Liability On behalf of myself and the minor, I hereby release, waive, and discharge Pinky’s Tee Box, its owners, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to the minor’s participation, including but not limited to injury, illness, medical expenses, disability, death, or property damage. 4. Medical Responsibility I accept full responsibility for any medical care or expenses that may arise from injury to the minor while using the facilities. I authorize emergency medical treatment if deemed necessary. 5. Indemnification I agree to indemnify and hold harmless Pinky’s Tee Box from any claims, damages, or expenses (including attorney fees) resulting from the minor’s participation or my failure to disclose relevant information. 6. Rules and Supervision I agree that the minor will follow all posted rules and staff instructions. I understand that supervision may be required and that Pinky’s Tee Box reserves the right to remove any participant for unsafe behavior. 7. Acknowledgment of Understanding * I have read this agreement, fully understand its terms, and sign it voluntarily on behalf of myself and the minor. I understand that I am giving up certain legal rights on behalf of the minor and myself. LIABILITY WAIVER AND RELEASE OF CLAIMS Pinky’s Tee Box Golf Simulator * I, the undersigned participant, acknowledge and agree to the following terms and conditions in consideration for being permitted to use the golf simulator and related facilities at Pinky’s Tee Box: 1. Assumption of Risk I understand that use of a golf simulator involves inherent risks, including but not limited to bodily injury, property damage, or death. These risks may arise from swinging golf clubs, being struck by golf balls, equipment malfunction, or the actions of myself or others. 2. Release of Liability I hereby release, waive, and discharge Pinky’s Tee Box, its owners, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action that may arise from my use of the facilities, including but not limited to injury, illness, medical expenses, disability, death, or property damage. 3. Medical Responsibility I acknowledge that I am solely responsible for any medical costs or expenses incurred as a result of injury sustained while using the golf simulator or facilities. Pinky’s Tee Box does not provide medical insurance coverage. 4. Indemnification I agree to indemnify and hold harmless Pinky’s Tee Box from any loss, liability, damage, or costs (including attorney fees) that may arise due to my participation or actions while using the facilities. 5. Fitness to Participate I certify that I am physically and mentally capable of safely participating in activities at the golf simulator and have not been advised otherwise by a medical professional. 6. Rules and Conduct I agree to follow all posted rules, instructions, and staff guidance. Failure to do so may result in removal from the premises without refund. Guest Responsibility: The person making this reservation represents that they are at least 18 years of age and accepts responsibility for all individuals attending under this reservation. The reserving customer agrees to inform all guests of the facility rules and safety requirements before play. By bringing guests into the facility, the reserving customer acknowledges that each guest is entering at their own risk and is expected to comply with all posted rules and safety instructions. The reserving customer accepts All financial responsibility for any damage to the facility, Guests or equipment caused by themselves or their guests. The Reservation Holder agrees to: • Inform all guests of the facility rules and safety requirements before entering the simulator bay. • Ensure that all guests follow posted rules and instructions. • Accept financial responsibility for any damage to the facility, equipment, or property caused by themselves or any member of their party. • Ensure that all guests understand that participation in golf simulator activities involves inherent risks, including but not limited to injuries caused by golf clubs, golf balls, slips, falls, or contact with equipment. The Reservation Holder acknowledges that all guests enter and participate voluntarily and at their own risk. To the fullest extent permitted by Oklahoma law, the Reservation Holder agrees to defend, indemnify, and hold Pinky’s Tee Box LLC harmless from claims arising out of the acts or omissions of the Reservation Holder or their guests, including claims for property damage or injuries caused by the Reservation Holder or members of their party. The Reservation Holder understands that this agreement does not waive or release legal rights belonging to another competent adult who has not personally agreed to this waiver. 7. Acknowledgment of Understanding * I have read this waiver carefully, fully understand its terms, and sign it freely and voluntarily. I understand that by signing this document, I am giving up substantial legal rights. VIDEO SURVEILLANCE AND RECORDING CONSENT AGREEMENT 
Pinky’s Tee Box By entering and using the facilities at Pinky’s Tee Box, you acknowledge and agree to the following: 1. Notice of Surveillance
I understand and acknowledge that Pinky’s Tee Box utilizes video surveillance and recording devices throughout the premises for purposes including, but not limited to, safety, security, and business operations. 2. Consent to Recording
I hereby consent to being recorded by video and/or audio surveillance systems while on the premises. This includes my likeness, image, voice, and actions. 3. Use of Recordings
I understand that recorded footage may be used for security purposes, incident investigation, staff training, marketing, or promotional use, unless otherwise prohibited by law. 4. Release of Liability
I release and hold harmless Pinky’s Tee Box, its owners, employees, agents, and affiliates from any claims related to the use of such recordings, including claims involving privacy, publicity rights, or defamation, to the extent permitted by law. 5. No Expectation of Privacy
I acknowledge that I have no reasonable expectation of privacy in common or public areas within the facility where surveillance is in use. 6. Compliance with Laws
 - Pinky’s Tee Box agrees that all surveillance practices will be conducted in accordance with applicable laws and regulations. 7. Acknowledgment of Understanding
By entering the premises, I acknowledge that I have read and understand this notice and consent to video and audio recording as described.

WE DO NOT REFUND FOR FORGETTING TO CANCEL A MEMBERSHIP! * Billing & Terms- Memberships will be billed monthly, and will automatically renew every month on the signup date. * Members can cancel their membership at anytime. * No refunds for partial months * Rollover Policy- Extra hours on memberships will not roll over to the next month. * Guests- Membership holders are not allowed to bring more than 4 guests at a time. * Members Must be present with guests at all times. * Members Cannot share their membership! * Membership holders are responsible for any and all damages the members or their guest cause.
